All surgeons advise what is best for their patients in their opinion.

In our experience, all the above activities have a potential of causing problems but no clear evidence that these are beneficial.

In my series, I have not noticed any long-term problems with swelling unless it was already expected and was discussed with the patient. If necessary we will discuss the postoperative management changes and you are of course welcome to discuss these during your meetings with the surgeon.

Most surgeons offering surgical Rhinoplasty on a regular basis would have considered the need for a preoperative Psychiatric Evaluation. Some surgeons believe that they can detect the warning signals and it is needed only when they are unsure of safe candidacy in a patient while some others believe that validated screening tools are adequate to avoid operating on patients where the injudicious offer of surgery may be harmful to the patient. We are at a stage where more data and frequent discussions will indicate the balanced approach to optimise formal evaluation based on guidelines.

We have a unique database of having completed Psychiatric Evaluation as a Mandatory requirement prior to offering Cosmetic Rhinoplasty. This is a prospective study that exposed a 4% failure of the Surgeon recognizing mild Body Dysmorphic Disorder (BDD) despite all the attention of this very group. It is well-recognized that interviews, screening, and Surgeons honed gut-feelings are not enough to detect marginal BDD in all cases.

As our experience with the Mild BDD group has increased, our early experience indicates that more detailed counseling, the involvement of patients own support circle and presence of good Insight allows safe and satisfactory surgical outcomes in this group. The long-term results will be important in this group to conclude that intervention in selected cases of mild BDD is safe.

3D Image Manipulation or Traditional High Resolution Photographs for my Rhinoplasty Practice

Digital Manipulation of photographs in a Rhinoplasty Practice is getting more commonplace but is by no means a universally accepted practice.

I have always found it most useful and it forms the central point of my discussions with all my patients. This is also my favourite tool to hone my art and I often pick interesting faces off Pinterest to study their aesthetics and practice what looks pleasing and what is achievable.

An excellent camera and the right lens allows excellent High Resolution Documentation of reasonable and manageable size.

My interest in 3D imaging of face goes back to the days of my Rhinoplasty Fellowship in 2006 when I explored its readiness and availability. In Fact me and my team presented our research on how a CT Scan of Sinuses can be used to generate 3D Imaging in 2007.

As the 3D imaging is becoming more affordable I can see its advantages in types of surgery where a significant volume reduction or augmentation is performed.

For my Rhinoplasty practice I think it is a gimmicky tool that adds very little as Photographs are a far better way to document, manipulate for discussion, compare Before & After, publish and present our High Resolution work.

Since Rhinoplasty is the only Cosmetic surgery I offer … It is not for me … for now.
